Home and Community Based Services offered by Eliza Jennings are designed to assist older adults who live independently in their homes. This service helps older adults maintain their independence and continue to live full, satisfying lives. It centers on home-based care managed by nurse practitioners licensed by the State of Ohio and accredited by the Joint Commission.

Eliza Jennings is the proud recipient of the Joint Commission’s Gold Seal of Approval, and the program is an in-network option accepted by most major insurers.

Wellness Clinics

homeEliza Jennings Home and Community Based Services are housed in our wellness clinics: HUD 202 residential communities in Cleveland, Lakewood and Bay Village, dedicated to over 500 lower-income older adult residents, and at The Renaissance Retirement Campus in Olmsted Township. The clinics exemplify Eliza Jennings’s dedication to providing superior services to individuals when and wherever needed. Our caregivers at the clinics are dedicated to ensuring residents have all of the options and resources they need to live independently for as long as possible.

At our on-site wellness clinics, certified nurse practitioners are available to help residents achieve optimal health. Qualified and experienced clinicians are available to diagnose and treat patients, as well as write prescriptions and provide wellness education and preventative monitoring.

Scope of Services

Caregivers employed by Eliza Jennings bring intermittent and part-time services directly to the individual requiring assistance, including skilled nursing care, health assessments, family education on senior health, visits by social workers and home health aides, and therapies for older adults with physical, occupational or speech therapy needs.

Eliza Jennings Home and Community Based Services are designed to address clinical issues as soon as they arise and in a familiar environment, providing a seamless coordination of care between caregiver and resident.

With expert care, we provide:

  • Disease management for patients with diabetes, chronic obstructive pulmonary disease (COPD), cardiac issues, and cholesterol problems
  • Situation management for patients recently hospitalized or placed on new medications, and for those requiring education on managing chronic illness
  • Transitional care for patients in recovery, requiring medication reconciliation, continued follow-up on clinical issues, and hospital-to-home transitions
  • Skilled home health for patients requiring nursing visits for wound or catheter care, progress assessments, rehabilitation, intravenous medication, and social service assessment
  • Caregiver support for residents who may require housekeeping assistance, companionship, and assistance with daily activities

All services are available 24 hours a day, seven days a week, and for any length of time. House calls are made exclusively by certified nurse practitioners, and registered nurses are available for patients as case managers capable of assisting with medications, appointments, injections, diagnostics, and basic support services.
